MRI Scan Assessment for Prostate Cancer

Multiparametric MRI (mpMRI) is now one of the most important tests in modern assessment for prostate cancer.  It can help identify in the diagnostic pathway or rule out suspicious areas within the prostate, reduce unnecessary biopsies in some men, and guide more accurate targeted biopsies when they are needed. Prostate cancer assessment in the UK usually begins with a PSA blood test, which is not a perfect test. 

An MRI can often be arranged quickly where the PSA is abnormal, giving faster answers and helping men move from concern to clarity without unnecessary delay. The MRI helps by showing whether or not there are areas suspicious for prostate cancer, avoiding unnecessary biopsy in some patients with high PSA levels but no concerning shadows on the scan, and targeted biopsy of abnormal areas, which can improve diagnostic accuracy. 

MRI is also used for monitoring early tumours during active surveillance, and treatment planning for focal therapy, robotic prostatectomy, and radiotherapy.

Is MRI a screening test?

This is one of the commonest questions men ask. At present, MRI is not yet routinely used as a blanket screening test for all men in the UK; it is usually recommended after an elevated PSA or in some cases to investigate causes of pelvic pain.

Who may benefit from a private prostate MRI?

A private MRI assessment may be helpful if:

  • Your PSA is raised or rising on monitoring or active surveillance.
  • You are a carrier of a BRCA gene mutation or have a family member with this gene mutation or have a strong family history of prostate, breast or ovarian cancer.
  • You are from a Black Caribbean or Black African ethnic group and are concerned about higher prostate cancer risk.
  • You have had a next generation prostate cancer test e.g polygenic risk score (PRS) or biomarker test e.g. 4K or Stockholm3 which is abnormal

What is the difference between multiparametric and biparametric MRI?

Multiparametric means that multiple parameters are used. Multiparametric MRI involves giving an injection of a contrast agent (Gadolinium), as well as other sequences (T1, T2, Diffusion-weighted) to help identify tumours within the gland.  Biparametric MRI omits the constrast agent injection, and there is some evidence to show that it does not make a major difference in the diagnosis of prostate cancer. 

However, whether or not it affects treatment decision and planning is not yet known.  It is popular in the NHS because the scan is quicker, and cheaper, so helps hospitals process patients more efficiently.

What is a prostate MRI scan like for patients?

A prostate MRI scan is usually a safe, painless and non‑invasive test that creates detailed images of the prostate without using ionising radiation. During the scan, you lie flat on a narrow table that moves into the MRI scanner, and the test usually takes around 20 to 40 minutes depending on the type of images needed. The scanner is noisy, with repeated tapping or buzzing sounds, so ear protection is normally provided, and some centres also offer music to make the experience more comfortable. Most men tolerate prostate MRI well, but it is important to keep still throughout the scan.

Some men worry about feeling enclosed during the scan. Mild claustrophobia is fairly common, and a small proportion of people find MRI difficult because of the partly enclosed design of the scanner, although reassurance, preparation and sometimes mild sedation can help. If you are anxious, prone to claustrophobia or have struggled with scans before, it is worth mentioning this in advance so the imaging team can talk through the options.

Who may not be able to have an MRI scan?

Not everyone can safely have an MRI, because the scanner uses a very strong magnetic field. Men with certain pacemakers, implanted defibrillators, cochlear implants, programmable shunts, neurostimulators, some aneurysm clips, or metal fragments in the eye or head may not be suitable for MRI, or may need additional specialist safety checks first. Safety depends on the exact device and whether it is MRI‑safe or MRI‑conditional, which is why detailed screening questions are always asked before the appointment.

MRI can also be difficult for men with severe claustrophobia or those unable to lie still or flat for the duration of the scan.

What are the alternatives if MRI is not possible?

If an MRI scan cannot be performed, there are still other ways to assess the prostate and investigate possible prostate cancer. These may include newer ultrasound technologies such as micro‑ultrasound, PSMA-PET/CT scanning or mapping prostate biopsy.
