It is often recommended for men early, slow-growing prostate cancers that is unlikely to spread in the foreseeable future. The primary goal of active surveillance is to monitor the cancer closely for as long as possible to avoide unnecessary treatment and its associated side effects unless without affecting cancer survival rates.

Why Active Surveillance?

Prostate cancer can vary in its aggressiveness from a “pussycat” cancer that grows so slowly that it may never need treatment to a “tiger” cancer that needs urgent treatment. For “pussycat” cancers, the risks and side effects of treatments such as surgery or radiotherapy may outweigh any potential benefits. Active surveillance offers an alternative that allows patients to maintain a good quality of life without affecting how long they live.

How Does Active Surveillance Work?

Active surveillance involves regular monitoring of prostate cancer through a series of routine tests and assessments. These generally include:

  • PSA Tests: Prostate-specific antigen (PSA) levels are measured every 3-4 months. A rise in PSA levels could indicate that the cancer is becoming more active.
  • MRI Scans: MRI scans are performed on a regular basis (e.g. annually) or may be recommended if there is a suspicion of growth based on a PSA rise.
  • Prostate Biopsies: Biopsies may be carried if the PSA and MRI indicates that the cancer may have progressed or become more aggressive.

 

The frequency of these tests is determined by the healthcare team based on individual circumstances, but they typically occur every 3-6 months.

Benefits and Disadvantages of Active Surveillance

  • Benefits: Men on active surveillance never require further treatment during their lifetime, thus avoiding potential side effects such as incontinence and erectile dysfunction and maintaining quality of life.
  • Disadvantages:  In some rarer instances, the prostate cancer may progress whilst on active surveillance and spread to other parts of the body excluding a patients from having a curative treatment.  Fortunately, this is an infrequent scenario.

Diet and Exercise

It is important to have a healthy lifestyle including eating a balanced diet and undertaking regular exercise whilst on active surveillance.  This is important so that you can be fit for treatments in the future if necessary. Also, there might be some dietary supplements that reduce the risk of the cancer progressing (e.g. Pomi-T, Vitamin D), although this is yet to be proven in trials.

When to Move from Surveillance to Treatment

Active surveillance is not a permanent solution for every patient. There may come a point when active treatment is recommended, for instance, if there are signs that the cancer is growing more quickly or becoming more aggressive. This decision is based on evidence gathered from PSA tests, biopsy results, and imaging. The goal is always to ensure that any progression is caught early, so effective treatment can begin while the cancer is still manageable.
